Step one: DBS
You can get a form from us by calling 0300 062 8163 or by emailing firstname.lastname@example.org Guidance on completing the DBS form can be found at www.homeoffice.gov.uk/dbs
You must send the completed DBS application form and original identity documents to us. You must also send a payment by cheque or card for £44 to cover the cost of the DBS check.
Step two: References
You will need to obtain and send us the following references:
- Financial reference from a suitably qualified person (Bank Manager/Accountant)
- Medical Reference
- Two personal references

Step three: Application form
The application form must be completed whether you are applying as an individual, Registered Manager and/or an organisation. Please complete all sections that are relevant.

Step four: Statement of Purpose, Patient Guide and index of policies and procedures
You must include your Statement of Purpose, Patient Guide and index of policies and procedures with your application. Templates are available below.
Step five: Submitting your application
Please email your completed application form and all other necessary information to email@example.com or you can post a hard copy to Registration Team, HIW, Welsh Government, Rhydycar Business Park, Merthyr Tydfil, CF48 1UZ.
