The legal requirements of the Private Dentistry Regulations 2008 and the Private Dentistry (Wales) (Amendment) Regulations 2011 must be met prior to and during registration.

Step one: DBS certificate

You must send us the completed DBS application form and original identity documents to the address below. You must also send payment by cheque or card for £44 to cover the cost of the check.

If you have an Enhanced DBS check that was issued within the last 3 years for the purposes of being included on a dental performers list, please include the original certificate with your application.

Step three: Application fee

The registration fee is £75 which can be paid by card or cheque. You will need to complete a registration payment form for either of these payment methods and submit with your application. The Guidance on Fees document provides further information.

Step four: Submitting your application form

Once you have completed the relevant forms and enclosed all documents needed, you can email them to or post in hard copy to our office.

Annual Registration

Once you are registered with us you will need to pay an annual fee of £75. This will be due on the anniversary of your registration each year. You can pay by cheque, (made payable to Welsh Government and posted to our office) or over the phone by card by calling 0300 062 8163.

When contacting us to make a payment please include your General Dental Council (GDC) number. If sending a cheque, please ensure your GDC number is clearly noted on the reverse of the cheque.
