Notifying Dental Incidents

Dental Practices must notify us when a particular event occurs as stated in The Private Dentistry (Wales) Regulations 2017.

Regulations 25 of The Private Dentistry (Wales) Regulations 2017 state that the registered person of a private dental practice must notify us about particular events that occur relating to patient safety.

How to report a notifiable event

When notifying us of a notifiable event, use the relevant notification form from the list below. Please give as much detail as possible when completing the form and enclose all supplementary documentation such as investigation reports you have commissioned. Further information on completing the forms is available in the guidance.

How to submit your completed notifiable event form

Completed forms will often contain personal or commercially sensitive information and deserve adequate protection. Forms must be submitted either electronically through Objective Connect. (If you do not have an Objective Connect account please contact HIW on 0300 062 8163) or in hard copy using a Recorded Delivery service.

What happens after you have reported an event

An acknowledgement letter/email will be sent to you once we have received your notification with a reference to be used on all future correspondence. All notifcations will be considered by us within 10 working days. We will contact you if we require further information. The information provided on the notifiable event forms enables us to assess a healthcare provider’s ability to comply with the 2017 Regulations and that patients are being appropriately safeguarded.
