This report is a review of the provision of mental health services provided to Mr B prior to April 2006.
Healthcare Inspectorates Wales have been commissioned by Welsh Ministers to undertake this external review of the provision of mental health care and treatment provided to Mr B prior to the homicide committed in 2006 to determine if there were shortcomings in the care and treatment Mr B received that may have influenced or led to the events of April 2006.
