This report is a review of the provision of mental health services provided to Mr G prior to May 2009.
Healthcare Inspectorates Wales have been commissioned by Welsh Ministers to undertake this external review of the provision of mental health care and treatment provided to Mr G prior to the homicide committed in 2009 to determine if there were shortcomings in the care and treatment Mr G received that may have influenced or led to the events of May 2009.
