Healthcare Inspectorates Wales have been commissioned by Welsh Ministers to undertake this external review of the provision of mental health care and treatment provided to Ms A prior to the homicide committed in 2005 to determine if there were shortcomings in the care and treatment Ms A received that may have influenced or led to the events of October 2005.
