Reviews - Patient surveys
As part of our annual reviews programme, we have committed to undertake a local review of the quality of discharge arrangements for adult patients from inpatient mental health services in Cwm Taf Morgannwg University Health Board (CTMUHB).
You should only complete this questionnaire, if you have been admitted and subsequently discharged from a mental health inpatient unit within CTMUHB.
If you are the family/ carer of a mental health patient then you can complete our other questionnaire.
We are currently undertaking a National review to assess the impact of any delay in assessment or treatment for patients. We are using the stroke pathway to assess the quality of a patient’s journey. We want to understand what is being done to mitigate any harm to those awaiting care and how the quality and safety of care is being maintained throughout the stroke pathway.

National Review of Patient Flow - Stroke Pathway
We want to hear from you if your role falls within:
Stroke Services, Patient Flow, Senior Management, Emergency Department, Discharge Staff or Site/Bed Management.
The Welsh Ambulance Service Trust play a key role in transporting and supporting stroke patients in the community into secondary care and your feedback is imperative to understand the full pathway.
Local Authorities play a key role in supporting discharge from hospital and supporting people in the community. Feedback from Local Authorities is imperative to understand the full pathway.
We are working in partnership with CIW (Care Inspectorate Wales) on this review as Social Care Providers; Care Homes (Nursing and Residential) and Domiciliary Support Services play a key role in supporting people in the community.
