This report is a review of the provision of mental health services provided to Mr E prior to August 2007.
Healthcare Inspectorates Wales have been commissioned by Welsh Ministers to undertake this external review of the provision of mental health care and treatment provided to Mr E prior to the homicide committed in 2007 to determine if there were shortcomings in the care and treatment Mr E received that may have influenced or led to the events of August 2007.
