This report is a review of the provision of mental health services provided to Mr F prior to December 2008.
Healthcare Inspectorates Wales have been commissioned by Welsh Ministers to undertake this external review of the provision of mental health care and treatment provided to Mr F prior to the homicide committed in 2008 to determine if there were shortcomings in the care and treatment Mr F received that may have influenced or led to the events of December 2008.
