Report of a Review in respect of Mr L and the provision of Mental Health Services, following a Homicide committed in October 2012

Healthcare Inspectorates Wales have been commissioned by Welsh Ministers to undertake this external review of the provision of mental health care and treatment provided to Mr L by Cardiff and Vale University Health Board prior to the homicide committed in 2012 to determine if there were shortcomings in the care and treatment Mr L received that may have influenced or led to the events of October 2012.
