This report is a review of the provision of mental health care and treatment provided to Mr M by Betsi Cadwaladr University Health Board (BCUHB), prior to committing a homicide in Tenerife, May 2011.
Healthcare Inspectorate Wales have been commissioned by Welsh Ministers to undertake this external review of the provision of mental health care and treatment provided to Mr M determine if there were shortcomings in the care he received that may have influenced and led to the events of May 2011.
